This article describes NACHA return rate thresholds, operational management practices, and ACH return codes within the FORTH system. It was developed for for financial administrators, compliance officers, and payment operations staff managing ACH payments and return thresholds in FORTH.

Overview
ACH return codes identify why a receiving bank returned an ACH payment, enabling clear communication regarding payment failures. NACHA enforces strict return rate thresholds to evaluate origination quality across the network and prevent problematic origination activity. Excessive return rates can lead to network penalties or operational restrictions on your account. FORTH provides monthly return rate tracking and code reference tools to help you maintain compliance and reduce payment failures.
Prerequisites
Standard user access to the FORTH CRM with payment processing and compliance reporting permissions.
How to Access
- Log in to the FORTH CRM.
- Click the Reports tab.
- Navigate to ACH Processing > Return Rate Reports.
How it Works
- Review NACHA return rate threshold categories to monitor compliance:
- Unauthorized Returns (0.50% Threshold): Covers debit entries returned under codes R05, R07, R10, R11, R29, and R51.
- Administrative Returns (3.0% Threshold): Covers debit entries returned due to administrative or account data errors under codes R02, R03, and R04.
- Overall Return Rate (15.0% Threshold): Applies to all debit entries returned for any reason, excluding RCK entries.
- Access your monthly FORTH return report to review your rolling 60-day return rate broken down by unauthorized, administrative, and overall returns.
- Implement operational practices to keep return rates below NACHA thresholds:
- Statement Transparency: Inform customers that FORTH will appear as the descriptor name on their monthly bank statement.
- Scripting Reviews: Periodically review enrollment scripts and welcome calls to reduce unauthorized initial debits.
- Payment Reminders: Issue payment reminders via telephone calls, text messaging, or email to mitigate insufficient funds (R01) returns.

RDFI Return Reason Codes
The following codes are used by Receiving Depository Financial Institutions (RDFIs) to indicate why an ACH entry was returned:
| Code | Description |
| R01 | Insufficient Funds - Available balance is insufficient to cover dollar value of the debit entry. |
| R02 | Account Closed - Previously active account has been closed by the customer or RDFI. |
| R03 | No Account/Unable to Locate Account - Account number structure is valid, but does not correspond to an open account. |
| R04 | Invalid Account Number - Account number structure is invalid or fails check digit validation. |
| R05 | Improper Debit to Consumer Account - CCD, CTX, or CBR debit entry transmitted to a consumer account without authorization. |
| R06 | Returned per ODFI's Request - ODFI requested RDFI to return the ACH entry. |
| R07 | Authorization Revoked by Customer - Consumer revoked authorization (must return within 60 days of settlement with signed affidavit). |
| R08 | Payment Stopped - Receiver of recurring debit stopped payment on a specific ACH debit. |
| R09 | Uncollected Funds - Sufficient ledger balance exists, but funds are uncollected or cash reserve is below debit entry value. |
| R10 | Originator Not Known and/or Not Authorized - Consumer advised Originator is not authorized to debit account (within 60 days with signed affidavit). |
| R11 | Customer advises not within Authorization Terms - Payment received for incorrect amount or on wrong date. |
| R12 | Account Sold to Another DFI - Entry destined for an account sold to another financial institution. |
| R13 | Invalid ACH Routing Number |
| R14 | Account-holder Deceased - Account holder, representative payee, guardian, or trustee is deceased. |
| R15 | Beneficiary Deceased - Beneficiary entitled to payments is deceased. |
| R16 | Account Frozen / Returned Per OFAC - Funds unavailable due to specific legal or RDFI action. |
| R17 | File Record Edit Criteria / Suspicious Entry with Invalid Account No. / Return of Improperly-Initiated Reversal |
| R18 | Improper Effective Entry Date |
| R19 | Amount Field Error |
| R20 | Non-Transaction Account - Entry destined for an account against which transactions are prohibited or limited. |
| R21 | Invalid Company Identification |

| R23 | Receiver Refused Credit - Receiver refused credit entry due to incorrect amount, litigation, or lack of authorization. |
| R24 | Duplicate Entry - Entry matches another transaction in trace number, date, amount, or data. |

| R29 | Not Authorized by Corporate Customer - Non-consumer Receiver notified RDFI that entry was unauthorized. |

What is the difference between an RDFI return code and a FORTH internal code?
RDFI codes (R-codes) originate from the receiving bank after an ACH entry attempts processing. FORTH internal codes (I-codes) are system-level rejections triggered before transaction submission to prevent processing errors.
How should you handle an Administrative Return (R02, R03, or R04)?
Update the customer bank account number and routing details in FORTH before attempting another payment entry.
